Describe the observation
Record what was found, where it appeared and how it was confirmed. Separate a reproducible issue from a question that still needs investigation. Use enough context for another person to understand the finding without guessing.
Explain the next step
Give each finding an owner and an actionable next step. Describe the condition that would show the issue has been addressed. Keep sensitive details in a restricted record, especially when they could reveal internal systems or unresolved weaknesses.
Review completion deliberately
A status change should follow a meaningful check. Record the result and any remaining limitations rather than simply marking an item done. An understandable action list helps the team focus attention and preserves a useful history for the next review. Public writing can discuss the method without disclosing private findings.
